- Do you struggle with managing critical feedback from coaches, managers, or supervisors, leading to extended periods of self-doubt and rumination over mistakes?
- Do you need a provider who specifically understands the unique culture, stressors, and demands of competitive or high-stakes environments and integrates that understanding into treatment?
Section 2: General Emotional & Psychological Well-being
- Do you experience symptoms of anxiety or depression that persist even when you are away from your sport or activity?
- Do you struggle with sleep disturbances, changes in eating habits, or low energy that are impacting both your general well-being and your athletic recovery?
- Do you have a history of a formal psychiatric diagnosis (e.g., ADHD, Depression, Generalized Anxiety Disorder, OCD) that you need help managing within the context of your demanding schedule?
- Are you currently recovering from a significant injury or setback and finding that the psychological toll, such as fear of a recurrence or loss of professional identity, is blocking your rehabilitation or progress?
- Do you feel a sense of professional burnout, cynicism, or emotional exhaustion that standard time off has not resolved?
Section 3: Weight Management & Physical Health Focus
- Do you often feel unhappy or depressed about your weight or physical fitness?
- Do you frequently feel embarrassed or self-conscious about your body in social situations?
- Do you avoid social gatherings or activities (e.g., swimming, gym, or parties) due to concerns about your weight?

- Have you previously used medication, therapy, or other interventions to help with weight loss?
Section 4: Your Needs & Logistics (Practice Fit Confirmation)
- Are you in a stable enough emotional state that you are unlikely to need immediate crisis intervention or emergency services during the course of outpatient treatment?
- (A "No" answer means you likely need a higher level of care than we can safely provide in an outpatient setting).
- Are you seeking an integrated approach that combines medical expertise (psychiatry) with performance psychology strategies and/or weight management support, rather than only basic motivational coaching?
- ("No" indicates a potential mismatch, as our approach is clinical and integrated).
- Are your current needs manageable within a standard outpatient setting (e.g., weekly or bi-weekly appointments), rather than requiring highly intensive daily support programs?
- ("No" indicates you may need a higher level of care, such as an IOP or PHP).
